DOB 15 July 2004

Was admitted to the Orphanage with the following diagnosis: brain damage related to hypoxia and ischemia, OU – microphtalm, OS – coloboma of retina, anemia 1st degree.

Born to young and healthy parents, first pregnancy, mother has rubella during pregnancy. Apgar score 8-9, weight at delivery - 3,400 g, length – 50 cm. Early neonatal period without any problems; had several flu and chicken pox, some developmental delay. Vaccinated according to schedule.

Neurologist (21.12.07) – mild delay in psychological and verbal development
Ophthalmologist (13.02.07, Ophthalmologic Clinic “Ocomed”)OD: microblepharon, microphtalm, blepharo- conjunctivitis OS: microphtalm, coloboma of retina, coloboma of n.ophalmicus

***In layman's terms, Tommy was born missing his right eye.***

Surgical treatment is currently not needed.

Recommendations: prevention of trauma of left eye, regular consultations with ophthalmologist.
